HISTORY OF THE FOUNDATION

  • The Fondation du CLSC-CHSLD des Patriotes was created in 1994 at the request of the board of directors of the CLSC La Vallée des Patriotes, one of the local community service centres (CLSC) in Quebec receiving the least funding at the time. The mission given the Foundation by the CLSC administration was to support the centre in acquiring the equipment and the services it needed to provide health care to its numerous clients wishing to maintain or recover their autonomy or requiring treatment for their physical and mental health care needs.

  • ln 1998, the Foundation expanded its mission to include clients of the Marguerite-Adam centre in Beloeil and the Montarville centre in Saint-Bruno, both residential and long-term care facilities (CHSLD), following the merger in November 1996 of the CLSC with the CHSLD Monts-et-Rivières.

  • ln June 2003, the Foundation was named the Fondation du CLSC-CHSLD des Patriotes.

  • ln July 2004, the CLSC-CHSLD des Patriotes was amalgamated with health-care institutions in Saint-Hyacinthe to become the Centre de santé et de services sociaux Richelieu-Yamaska (CSSSRY).

  • Despite the merger, the Foundation continues to serve only the users of the CLSC des Patriotes and the Marguerite-Adam and Montarville facilities.
